Clinical evaluation of a newly designed orthodontic tooth brush - A clinical study |

CS Saimbi, Jha Thulika, AK Mehrotra, Kaushal Shalini In this study, a clinical trial was conducted with a newly designed orthodontic tooth brush, on ten subjects having fixed orthodontic appliances. In this newly designed tooth brush, the bristles of anterior portion of the head of the tooth brush are elongated and tapered towards the tip of the bristles and rest of the bristles are shorter than the anterior bristles. The inner bristles are further shorter than outer bristles row, providing a groove in the center of the brush. This groove is designed to engage the brackets. The elevated anterior bristles portion of the brush gets engaged in interproximal areas and cleans these areas.
In this study, the newly designed orthodontic tooth brush is compared with an ordinary tooth brush. Results of this study show that the newly designed orthodontic tooth brush is superior in its cleaning efficiency as compared to the ordinary tooth brush. The results show that plaque removing capacity of orthodontic tooth brush is nearly 95-99%. |

Comparative evaluation of Subepithelial Connective Tissue Graft (SCTG) and Acellular Dermal Matrix Allograft (ADMA) in the treatment of localized gingival recession- A clinical study |

AB Tarun Kumar, DS Mehta Gingival recession is the one of the commonest clinical features of chronic periodontitis and poses a great esthetic and functional problem to the patient. Several treatment modalities have been described in the dental literature to treat the localized gingival recession. In the present study, the efficacy of subepithelial connective tissue graft (SCTG) and the acellular dermal matrix allograft (ADMA) was evaluated and compared in the treatment of localized gingival recession. Nine patients with bilateral identical Miller's class I or II were selected and randomly assigned to treatment with either SCTG or the ADMA. Six months follow-up result showed significant clinical outcome in both the treatment groups in terms of improvement in recession depth, root coverage percentage, probing depth, clinical attachment level and the width of keratinized tissue. However on comparison between the two groups, no significant difference was found in relation to any of the parameters. Hence, it can be concluded that both the techniques (SCTG & ADMA) are equally effective in the treatment of localized gingival recession and restoring the esthetic and function. |

Use of autologous platelet - Rich plasma in the treatment of intrabony defects |

Sharath K Shetty, AR Pradeep, Vijay L Deshmukh, Aneesha Acharya Platelet Rich Plasma (PRP) has been combined with autologous bone or bone substitutes and used for periodontal regeneration. The clinical efficacy of this combination has been noted. However, it remains questionable whether this clinical efficacy is due to the PRP or the bone graft material alone with which PRP is used in combination. Objective was to assess the clinical effectiveness of autologous PRP alone in treating intrabony defects in humans. 6 patients were recruited having interproximal intrabony defects with pocket probing depth ≥ 5 mm at re-evaluation following initial therapy, and angular osseous defect depth ≥ 4mm as assessed by using Spiral Computed Tomography. Autologous PRP gel was obtained by mixing autologous PRP with autologous thrombin and placed in the intrabony defects after defect debridement. At 6 months postoperatively, mean probing pocket depth (PPD) noted at baseline (7± 1.27)mm reduced to3.67± 1.03 mm, clinical attachment level (CAL) gain was 3.33± 0.51mm, both of which were statistically significant. Mean distance from cemento-enameljunction (CEJ) to base of the defect (BOD) at baseline was 8.55 ± 0.89 mm. At 6 months it reduced to 6.58 ±1.13 mm showing a defectfillof 1.96±00.32 mm, which was statistically significant. Thepercentage of defect fill noted was 23.01 % and mean defect resolution was 2.05 ±0.30 mm, both of which were statistically significant.
Treatment of intrabony defects by autologous PRP gel alone caused significant soft tissue clinical improvement as well as hard tissue defect fill as evidenced by SSD view in spiral computed tomography. |

Use of transitional implants for immediate loading in mandibular complete dentures- A case report |

Seema Bhoosreddy, Vijay Deshmukh, Amrita Kishore After the placement of implants in areas in which several teeth have been lost, both the clinician and the patient face many difficulties, particularly during healing. If no prosthesis is provided, the patient's quality of life suffers. If a removable prosthesis is provided for optimum mastication and speech, many complicated adjustments of the denture may become necessary during healing, and the possibility of osseointegration failure increases. It has been reported that for implants to become osseointegrated, they must heal in the absence of functional loads for 4 to 6 months. To address the need for undisturbed healing and patient demand for uninterrupted immediate function and esthetics, the transitional implant system has been developed. This case report describes the use of transitional implants to support a removable mandibular overdenture. The transitional implant system is a sound and economical method of immediate patient restoration that allows for the protected healing of submerged implants. |

Implant placement and immediate loading in aesthetic zone- A report of three cases |

Vijay Deshmukh, Hiral Jhaveri, Amrita Kishore Single teeth replaced by implants in the aesthetic zone are one of the most challenging situations faced by a clinician. Recent studies have encouraged a progressive shortening of the healing period for single-tooth implants with immediate implantation and immediate loading being proposed for the aesthetic zone in the maxilla. The present report describes three cases with single tooth replacement in the anterior maxillary region by implant therapy with flapless technique followed by immediate provisionalization of the implant with all-ceramic crown. The patients exhibited no clinical or radiologic complications 1 year after loading. Implant placement and immediate loading of single tooth can provide an attractive treatment option in the aesthetic zone. Careful assessment must be made of mucosal and bone volumes in relation to implant placement for optimum aesthetic outcome. |

Effects of alkaline biocides on enterococcus faecalis - An in vitro study |

Lotika Beri, Shivangi Kumar, Shalini Aggarwal The purpose of this study was to do a comparative microbial assessment of disinfecting potential of Calcium hydroxide and Bioactive glass against Enterococcus faecalis when used as an intracanal medicament.
Thirty single rooted premolars undergoing extraction for orthodontic treatment were selected. Root canals of teeth with fully formed apices were instrumented using size 40 K file 1 mm short of canal length and irrigated with 2.5 % sodium hypochloride and EDTA. Teeth were then suspended in tryptic soya broth (TSB) and autoclaved. Positive control and study teeth, were inoculated with Enterococcus faecalis for two weeks in tryptic soya broth. Subsequently teeth were dressed with Calcium hydroxide or Bioactive glass (BAG) suspension for 10 days. Dentin samples were obtained from teeth using ISO size 45 H file to working length, cultured in tryptic soya agar and transferred to TSB for detection of growth by checking the turbidity of broth.
Unpaired t test and ANOVA test were used for statistical analysis. Both the alkaline biocides did not show complete elimination of Enterococcus faecalis. Calcium hydroxide had a stronger antibacterial effect than Bioactive glass.
Calcium hydroxide was an effective disinfectant. |

Perforation repair with artificial floor technique - A microleakage study |

Many materials and techniques have been tried to repair the furcation perforation. The purpose of this study is to evaluate the effect of calcium sulfate and hydroxylapatite matrix on the sealing ability of resin modified glass ionomer cement to repair the furcation perforation. The result of the study showed that the microleakage was least when perforation was repaired with resin modified glass ionomer cement over hydroxylapatite used as an internal matrix. And it was maximum when perforation was repaired with resin modified glass ionomer cement without any internal matrix material. |

Evaluation and comparison of biological cleaning efficacy of two endofiles & irrigants by microbial quantification- In vivo study |

Success of root canal therapy is majorly achieved by proper cleaning and shaping. However elimination of bacterial contaminants as well as necrotic debris of the canals requires the adjunctive use of irrigants.
To achieve a satisfactory biological and mechanical cleaning proper selection of endodontic instruments and irrigants is necessary.
In this study we are comparing and evaluating cleaning efficacy of endofiles (K-Flex files and Hand protapers) & root canal irrigants (3% Sodium Hypochlorite and Chlorhexidine2%) by microbial quantification. Root canal samples were collected in autoclavable bottles containing transport media (nutrient broth) and samples were cultured in tryptose soya agar at incubation temp of 37 0 c for 24-48 hrs and colonies were counted with digital colony counter.
The purpose of this study is to aid the clinician to select the proper instrument and irrigant which minimics the failure rate of root canal treatment for the benefit of patient. |

Efficacy and safety of dexketoprofen tablet in treatment of acute dental pain in Indian patients  |

Pankaj Chivte Background: Dexketoprofen is the pharmacologically active enantiomer ofracemic Ketoprofen, and is indicated for the treatment of acute pain. Objective: To record the efficacy and safety of Dexketoprofen 25 mg Tablet in treatment of acute dental pain.
Material and Methods: This was an observational, prescription event monitoring study in clinical setting. Indian male or female patients between 18-65 years of age who would be undergoing oral surgery consisting of extraction of the impacted or semi-impacted third molars or have acute dental pain and willing to comply with study were included. Patients were treated with Dexketoprofen Tablet 25 mg (Infen-25) every 8 hours preferably 30 min before meal. Efficacy was analyzed by decrease in pain on VAS (Visual Analogue Scale) and investigator's overall assessment of efficacy of therapy. Adverse events, if any, were noted. Results: Data of 3318 patients were analyzed. Mean baseline pain intensity on VAS decreased from 7.74 to 7.02, 5.48, 4 22, 3.31 and 2.52 at 10, 20, 30, 45 and 60 minutes, respectively, after drug administration. Investigators' overall assessment for efficacy was good to excellent in 96.5% patients, while investigators'Dexketoprofen was very well tolerated.
Conclusion: Dexketoprofen Tablet provided rapid onset of action, with good tolerability and very high patient and investigator satisfaction. |
